a. Permohonan

b. Foto copy KTP

c. Rekomendasi Camat

d. Foto copy Akte Pendirian Perusahaan (apabila berbentuk badan usaha)

e. Foto copy sertifikat tanah / perjanjian sewa tanah

f. Foto copy HO

g. Fotocopy IMB

h. Foto copy Dokumen Amdal/UPL/UKL

i. Pas foto 4 x 6 sebanyak 3 lembar
